Flu Vaccine Clinic for students and staff coming to TA in October
Thornton Academy is hosting a Flu Vaccine Clinic for TA students (grades 6-12) and staff on October 7, starting at 8 am, on the stage in the gymnasium.
Students will need to have a completed consent form, signed by a parent/guardian, to receive the vaccine. Consent forms are also available in the Nurse's Office. All consent forms must be returned to the nurse by September 26.
Once we receive a completed consent form, we will email the parent/guardian a questionnaire, which is required by MaineHealth to receive the vaccine.
